If you are a US citizen and planning to visit Ghana, you need to apply for a travel visa from the Ghana Embassy.  Visa requirements vary depending on the purpose of your trip.  Here are the requirements for tourists: The Ghana Embassy requires you to complete 4 copies of their visa application form. You can photocopy the application, BUT each form must have your original signature on it. So, if you don't want to fill out the form 4 separate times, fill it out but don't sign it, make copies and then sign each copy individually.
